shopping cart total your order, this form can then
be printed and faxed without completing an online
transaction. Include the book title and author (or order
or ISBN number), credit card number and expiration date,
the name as it appears on the card, and indicate if it's
MasterCard, VISA, or American Express. List your bill-to
address, and if different, your ship-to address. Please
also include a daytime phone in case we have questions
about your order.

Delivery: Orders are usually shipped via UPS
Ground-customers should receive their orders in 7-10
days. If you need faster service, see our web site or
call for types and costs of expedited shipping
available.
Returns/Defective Copies: If you're not happy
with your selection, please return the book for a full
refund (including shipping and handling). If you paid
via credit card, we will credit that credit card; if you
paid via check or money order, we will issue you a
check.
If a book is defective, simplify notify us and we'll
send out a free replacement copy. If the book's dust
jacket is damaged, we will gladly replace it free.
